Service Agreement

- Introduction
This Service Agreement (“Agreement”) is entered into between Cure & Travel by Horizonius Ltd. (“Company,” “we,” “us,” or “our”) and the client (“you” or “your”), effective upon your acceptance by checking the box below. This Agreement outlines the terms and conditions governing the medical tourism and travel services provided by Cure & Travel by Horizonius Ltd.
- Services Provided
2.1 Cure & Travel by Horizonius Ltd. acts as an intermediary facilitating medical tourism services, including but not limited to hair transplants, dental care, and other medical procedures.
2.2 We coordinate with licensed third-party medical providers, accommodation services, and travel facilitators to offer you a comprehensive experience.
- Privacy Policy
3.1 You consent to the collection, use, and processing of your personal data as outlined in our Privacy Policy.
3.2 Your information will only be shared with relevant third parties for the purpose of service delivery and will be handled in compliance with applicable data protection laws.
- Payment and Deposit Requirements
4.1 You agree to pay a non-refundable deposit as specified during the booking process to secure your services.
4.2 Full payment must be completed by the deadline communicated to you. Failure to make timely payments may result in the cancellation or suspension of services (see Cancellation Policy).
4.3 Any additional costs incurred due to changes in your itinerary, special requests, or medical needs must be paid promptly upon notice.
- Cancellation Policy
5.1 Deposits are non-refundable under all circumstances as per our Cancellation Policy.
5.2 Cancellation requests must be submitted in writing. Refunds, if applicable, will be assessed based on incurred costs.
5.3 Third-party costs, including but not limited to accommodation and travel expenses, are non-refundable.
- Code of Conduct
6.1 You agree to maintain respectful and professional conduct while interacting with our staff, service providers, and other clients as per our Code of Conduct.
6.2 Any inappropriate behaviour may result in the immediate termination of services without refund (see Cancellation Policy).
- Travel Documentation and Compliance
7.1 You are responsible for ensuring compliance with all travel documentation, including valid passports, visas, and any health-related requirements.
7.2 Cure & Travel by Horizonius Ltd. is not responsible for issues arising from incomplete or invalid travel documents.
- Medical Disclaimer
8.1 Cure & Travel by Horizonius Ltd. acts solely as a facilitator and is not responsible for medical procedures or outcomes.
8.2 All medical procedures are performed by independent third-party healthcare providers who are solely responsible for your treatment and post-care. You agree to release us from any liability related to medical outcomes or complications.
8.3 We do not provide medical advice. You should seek independent medical advice before undergoing any procedure. All treatments are carried out by licensed third-party providers who are solely responsible for your care and outcomes.
- Dispute Resolution
9.1 In the event of a dispute, both parties agree to seek resolution through informal negotiation.
9.2 If the dispute remains unresolved, the parties agree to submit to formal mediation or arbitration as outlined in our Dispute Resolution Policy.
- Limitation of Liability
10.1 Cure & Travel by Horizonius Ltd. is not liable for direct, indirect, incidental, or consequential damages resulting from medical procedures, travel delays, or third-party services.
10.2 Our liability, if any, shall not exceed the amount you paid for the services under this Agreement.
- Acknowledgment of Responsibilities
By accepting this Agreement, you confirm that:
11.1 You have read and understood all terms and conditions, including the Cancellation Policy and Dispute Resolution.
11.2 You accept responsibility for complying with all travel and health-related requirements.
11.3 You acknowledge that Cure & Travel by Horizonius Ltd. is an intermediary and is not responsible for medical outcomes.
- Governing Law
12.1 This Agreement shall be governed by and construed in accordance with the laws of England and Wales.
12.2 Any disputes arising from this Agreement will be subject to the exclusive jurisdiction of the courts of England and Wales.
